A psychiatrist, Dr. Ian Lazarre (John Savage), sinks into depression and drinking after his wife's death in an alcohol-related car accident, though he has remarried to a woman who looks just like his dead wife. He is talked into going to a rehab center that specializes in using experimental drugs. But the head of the institution, Dr. Renquist (Malcolm McDowell), is out to get Ian; in fact, Renquist murdered Ian's first wife—and staged the car accident—out of jealousy and now intends to take care of Ian.
