This prequel to "Carlito's Way" relates the early years of Puerto Rican Carlito Brigante (Jay Hernandez) as he begins his rise in the 1960s' drug world of Harlem. After being released from prison, Carlito joins his prison friends Afro-American Earl (Mario Van Peebles) and Italian-American Rocco (Michael Kelly), and they begin their drug empire. They hook up with Mafia lord Lerob "Hollywood Nicky" Barnes (Sean "P.Diddy" Combs) and are on their way. But problems lie ahead with Carlito's girlfriend Leticia (Jaclyn DeSantis), Earl's family, and the Mafia.
