In this follow-up to the TV series, the alien "Newcomers" from the planet Tencton and humans coexist in L.A. Meanwhile, the "Purists" have developed a virus to infect them. But more problems arise when Ahpossno (Scott Patterson) is sent from Tencton to capture the "Newcomers" and take them back to a world of slavery.

This story uses flashbacks to previous "Saw" films as Hoffman–while being pursued by Agent Straum (Scott Patterson)–now becomes the one using mechanical contraptions on victims that result in either mutilation or death.
