A psychopathic, 13-year-old orphan Rynn (Jody Foster), lives in a large house with a "father" who never seems to be home and nobody has seen. The fact that Rynn also gets very nervous when the subject of her basement is discussed adds to the chilling mystery. Nosy neighbor Mrs. Hallet (Alexis Smith) ends up being knocked down the basement stairs when she looks for evidence of Rynn's father being at home. Meanwhile, Mrs. Hallet's pedophile son Frank (Martin Sheen) is also curious and presses Rynn for answers–all of which leads to a nasty confrontation with Rynn's boyfriend Mario (Scott Jacoby).

"The Stranger" (Charles Bronson) is an amnesiac who was found disoriented on a beach with the body of a dead woman. Laurence Jeffries (Anthony Perkins) is the doctor on duty when the Stranger is brought to the hospital, and he has a "different" method of treatment. Jeffries takes the Stranger home, hypnotizes him, and gives him instructions to kill Jeffries' wife Frances (Jill Ireland) and her lover Paul (Henri Garcin). The Stranger, in his hypnotized state, sets off to do the killings, but will Jeffries change the plans before both are killed?
