Stand-up comedian George Simmons (Adam Sandler) learns that he suffers from a rare form of leukemia and has less than one year to live. George meets young Ira Wright (Seth Rogen) who dreams of becoming a comedian, and he offers Ira the job of working as his assistant and performing as his opening act. Meanwhile, George tries to reunite with his former girlfriend Laura (Leslie Mann), but her husband Clarke (Eric Bana) does not agree to the plan. Now, George learns that his disease has gone into remission, and he reevaluates his lifestyle and decides to find what will really give his life meaning.

Entertainment journalist Alison Scott (Katherine Heigl) celebrates a promotion by drinking too much and having a one-night stand with unemployed slacker Ben Stone (Seth Rogen) in her uptight sister Debbie's (Leslie Mann) and her husband Pete's (Paul Rudd) guest house. That would have been the end of the affair–until two months later when Alison discovers that she is pregnant. She decides to continue the pregnancy and keep the baby. In the process, Alison tries to work out differences she has with an immature Ben. But Alison soon learns that forming a lasting relationship with Ben will prove more difficult than making a baby with him.
