Barbara keeps them drugged while she feverishly tries to heal their wounds. And they don't seem to be Barbara's only "patients."

He also saves the day in Tokyo by fighting crime. Still lots of gore. Example: "A decapitated man dances a hillbilly jig while his neck is spurting blood" (Troma Movie Guide).
