In this fourth offering of "The Final Destination" series, Nick O'Bannon (Bobby Campo) has a premonition of events that lead to NASCAR car race crashes that kill Nick and his friends in the stands. Nick persuades his friends to leave just before his premonition becomes a reality, and cars crash into the stands. But, Nick's premonitions continue, and the "survivors" are killed in brutal ways leaving Nick struggling to cheat his own "final destination."

This horror tale involves three overlapping stories. In the first, "Crazy in Love," Ben (Justin Welborn) and his married lover Mya (Anessa Ramsey) are in Terminus City where they are awakened by a broadcast signal that turns ordinary people into murderers, and Mya encounters them on her way home. The second, "The Jealousy Monster," is about party hosts Anna (Cheri Christian) and Ken (Christopher Thomas) and their guests who experience murder along with the broadcast signal. In the third, "Escape from Terminus," Ben and Clark (Scott Poythress) search for Mya while technology is causing their world to crumble.
