Gangster John Sedley (Mickey Rourke) is badly disfigured when his partners–Sunny and Rafe (Ellen Barkin and Lance Henriksen)–double-cross him during a heist. John is sent to prison where, as part of a rehabilitation program, he is given plastic surgery. Now John is unrecognizable to anyone who once knew him, and, after being freed, "Johnny Handsome" seeks revenge on his old gang members.
