Psychiatrist Bill Crushank (Jeff Fahey) loses an arm in an auto accident and receives a new arm transplanted from an executed psychopathic killer. Bill soon starts having nightmares, and the arm seems to have a will of its own. He seeks out two other recipients of the killer's limbs and finds that both have some of the same symptoms. After the other two are brutally murdered and their transplanted limbs taken, Bill is confronted by the killer–none other than the remnants of the executed man.

It works, and one of the small demons appears. At first, things are great as wishes are granted, but things go sour and Terry must again fight the creatures while trying to re-close the gate.
