Just an average guy, Tom Stall (Viggo Mortensen) lives in Millbrook, Indiana, where he owns a diner. He is married to a lawyer, Edie (Maria Bello), and they have two children (Ashton Holmes and Heidi Hayes). Life changes for Tom after he stops a robbery at his diner by grabbing one of the criminal's guns and killing both robbers. During the ensuing news coverage, the media thrusts Tom and his family into the spotlight, and a one-eyed thug from Philadelphia, Carl Fogarty (Ed Harris), identifies Tom as a killer from 15 years ago and leaves for Millbrook to confront him. The film received two Academy Award nominations, including Best Supporting Actor (William Hurt).

As a fugitive on the run from the U.S. Military and Betty's father General Thaddeus "Thunderbolt" Ross (William Hurt), Banner is trying to find a cure for his condition that turns him into a giant green monster–The Incredible Hulk. The General does track down Banner and sends Emil Blonsky (Tim Roth) to Brazil to capture him. But, Banner turns into the giant green beast and escapes. Blonsky sees the change in Banner and wants to obtain the same powers for himself. He undergoes his own monster metamorphosis and becomes the Abomination. Now, the pursuit of "giant creature" by "giant creature" ensues.

This story uses flashbacks to previous "Saw" films as Hoffman–while being pursued by Agent Straum (Scott Patterson)–now becomes the one using mechanical contraptions on victims that result in either mutilation or death.

Strong, silent Smith (Clive Owen) encounters a pregnant woman (Ramona Pringle) who is being attacked by a thug. He intervenes and ends up delivering her baby (Sidney & Lucas Mende-Gibson) in the midst of a gun fight. The woman dies, and Smith escapes with the infant in tow. After figuring that the baby is the target, Smith takes the infant to a lactating prostitute (Monica Bellucci) to serve as a wet nurse. Now, Smith hunts down the criminal mastermind Hertz (Paul Giamatti) to find out just why he wants the infant killed. And gun battles ensue throughout the chase.
