After an absence of 150 years, a vengeful Tooth Fairy reappears in the town of Darkness Falls and terrorizes Kyle (Joshua Anderson) after he opens his eyes and sees her. Kyle returns to Darkness Falls to face the Tooth Fairy that caused his traumas of the past.
