Sheriff David Dutton (Timothy Olyphant) is enjoying life in his peaceful town of Ogden Marsh, Iowa, when toxins infect the citizens and turn them into violent psychopaths. Total chaos ensues, and the government steps in. The town is quarantined, and armed, gas-masked soldiers arrive on the scene to separate the sick from the healthy. Anyone who tries to escape is shot and killed. When David's pregnant wife Judy (Radha Mitchell), who is the town doctor, runs a fever, she is quarantined. David rescues her, and now David, Judy, Deputy Sheriff Russell Clank (Joe Anderson), and nurse Becca Darling (Danielle Panabaker) struggle to find the source of the toxins while eluding their infected neighbors and the government forces.

Treasure hunters (Matthew McConaughey and Steve Zahn) board a yacht in search of a lost, Civil War ironclad ship loaded with gold that they believe may have made its way across the Atlantic to West Africa. Along for the ride are two World Health Organization physicians (Penelope Cruz and Glynn Turman) who are tracking down the source of a plague.
