New Orleans Detective Danny Fisher (John Cena) stops a thief, Miles Jackson (Aidan Gillen), from committing a heist during which Miles' girlfriend is accidentally killed. Miles kidnaps Danny's girlfriend Molly Parker (Ashley Scott) and demands that Danny complete 12 tasks–12 rounds–in one day in order to save Molly's life.

After playing the game Stay Alive, Loomis (Milo Ventimiglia) and his friends (Billy Slaughter and Nicole Opperman) are murdered in the same manner they were killed in the game. Loomis' sister gives the game disk to Hutch (Jon Foster) who invites his friends (Jimmi Simpson, Samarie Armstrong, Sophia Bush, and Frankie Muniz) to come to his loft and play the game. Hutch's boss, Miller Bates (Adam Goldberg) joins in the game playing remotely from his office. The next day, Miller is found dead–murdered in the same manner he was killed in the game. Now, the four survivors realize that they must uncover the game's secret become they become victims.
